At Mount Desert Island Hospital Care Management is both a concept and a department. The concept is a simple one: People’s health improves if their care is coordinated and if it includes attention to physical, mental, emotional and social issues that might interfere with healing. This is especially important for people dealing with chronic health issues. The department is far from simple. Its challenges range from providing diabetes education and helping with discharge planning to assisting with Mainecare applications, coordinating medical services at home and hosting flu clinics.
The Care Management team is comprised of seven registered nurses and two licensed social workers. These friendly, informed and dedicated individuals are there to help you achieve better health outcomes by coordinating your care, helping you access additional services and assisting you in navigating the healthcare system.

- Anti-coagulation clinic for patients using blood thinners
